Nassau Chest Physicians, P.C. is a medical practice dedicated to providing excellent and compassionate care to patients with lung diseases and breathing difficulties. We have been serving the Long Island community for the past 20 years and will continue to do so in the future. We have grown to our present size of eight board certified physicians, two nurse practitioners, and two office locations.
As we enter the 21st century, we have created this web site to broaden access to our expertise and improve communication with our current patients. We hope that through education and increased access to information, our patients will be able to gain more control over their illnesses and be able to live healthier lives.
The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current insurance information.
